DIGITAL TRANSFORMATION OF HEALTHCARE
The first studies and publications devoted to the use of information technologies in the health care of the USSR appeared in the 60s of the last century. Since that time, our country has formed its own scientific school and a market for a wide variety of software products intended for use in medicine and healthcare. Since 2011, a number of major federal projects in the field of healthcare informatization have been implemented, which, in general, made it possible to provide infrastructure and basic equipment, communication channels and software for the lion’s share of medical organizations, connect them into a single secure information network and ensure the exchange and accumulation of data on the work of the health care system of the Russian Federation. The article presents a description of the history of the development of these processes, as well as the current results of informatization of the healthcare industry in Russia.
Prerequisites, creation, place and role of vertically integrated medical information system “Obstetrics and gynecology” and “Neonatoloy” (VIMIS “AKiNEO”, System) the purpose of which is to increase the efficiency of managing the vertical of medical care organization by providing prompt and comprehensive analysis of information, improving the qualifications of medical workers, as well as improving the quality of medical care to patients by improving information support at all levels in the digital transformation are considered in the article. VIMIS “AKiNEO” receives in the form of structured electronic medical documents, processes and aggregates primary data from external medical information systems. Thus, the principle of a single input of information and its further multiple use is observed. Components, working principles and current state of the System are described in details, including the results using the severe maternal morbidity register. Examples of clinical situations in which the registry played a significant role in saving life and health of patients are given. Positive effects of the System influence on the obstetric services both at the regional level and at the level of the whole Russian Federation are shown. The problems encountered during creation and implementation VIMIS “AKiNEO” in Russian digital healthcare system are presented.
Creating a unified information system in Russia is not a trivial task. The main difficulty is not in geographical or cultural features, but in the process of digitalization of the existing system. An important transformation tool is the involvement of professionals in the process of setting goals for system design, as well as testing the results.
The purpose of the work is to describe the concept of vertically integrated medical information system (VIMIS) “traumatology and orthopedics” as a digital tool for effective management of the provision of care by profile.
Materials. The existing projects of a VIMIS, the routing schemes for patients with injuries and diseases of the musculoskeletal system, the level of organization and provision of medical care in the regions are analyzed.
Results. The concept of management of medical care by traumatology and orthopedic the management scheme of the VIMIS project for the profile of traumatology and orthopedics (TO) were developed, an agreement was signed on the scientific and methodological interaction of participants. The project is aimed at the digital transformation of the called tasks, increasing the “digital qualification” to solve the search for tasks in the field of quality control of specialized medical care, efficient use of resources, substantiation of scientific problems. The difference between the profile of “traumatology and orthopedics” is determined by the fact that with the help of VIMIS TO a problem of different directions is solved, including indicators that can be incoherent.
Discussion. The project to create a vertically integrated medical system in the field of “traumatology and orthopedics” as a digital transformation of specialized care was based on an analysis of the most successful technological and organizational solutions: modern evidence-based models of treatment and care; artificial Intelligence; intelligent planning and treatment risk stratification; remote patient monitoring and the implant technology industry; advanced professional education platform.
Conclusion. The effectiveness of the model should be evaluated for the prospect of piloting with the integration of various regions medical information systems and its verification in the regions. Moreover, it is necessary to explore the sharing of large data sets and lay out the features of their use, given the differences in the nature of the cooperation of consortiums.
An overview of the architecture, regulatory documents and Internet sources on the digital transformation of occupational medicine, state IT platforms, the architecture of interdepartmental, interdisciplinary information interaction of IT systems, the formation of large user data, the use of risk management in functional personalized 4P medicine is presented. The principles of the formation of a unified information and analytical system “Occupational Medicine” are described.
ACTUAL ISSUES OF MEDICAL EDUCATION
Recent trends in intense evolution of information technologies and improving of medical education increases the availability of advanced training for doctors. These changes promote development of health care and society. The purpose of educational process modification is to create an environment for students to apply modern technologies, which allows students to remain interested in the disciplines being studied. Technological advancement results in creating of multimedia technologies that are currently being used to improve learning outcomes. The availability of multimedia technologies, digital content, and software simplify educational materials and make it more efficient for the learning process, and therefore empowers students.
The article provides an overview on currently used educational programs for medical residents, as well as an analysis of the sociological and psychological characteristics of modern students in the context of rapidly developing digital technologies. The article describes the current issues of the educational process implementation, highlights the complexity of the work of medical organizations and universities. The paper discusses the existing methods of modernizing education that meet modern trends in the development of medicine. The study also offers some important insights into using the innovative technologies by foreign colleagues. This research proposes a new methodology of integrating virtual reality into dermatovenereologists and cosmetologists training programs for optimizitation the educational process, standardization students and residents knowledge control, and improvement the practical training of healthcare personnel.
PUBLIC HEALTH AND HEALTHCARE
The prospects for the modernization of the Russian health care system are inextricably connected with expanding opportunities of the civil society to have an impact on the activities of health care organizations and state policy making in the field of health protection and promotion. At present, public organizations have become an integral part of a modern democratic state, ensuring interaction between the public sector and civil society, helping citizens to convey their opinions on health protection and health care development to government bodies, the professional community and providing assistance in decision making and implementation.
The ALL-RUSSIAN NATIONAL FRONT is an example of a large-scale public platform carrying out important functions in assisting the state in the implementation of key policy areas in the field of public health and health care. Public organizations empower civil society and promote volunteer movements, contributing to the involvement of citizens in the process of identifying problems in the healthcare sector and their participation in direct public control over the implementation of decrees and instructions of the President of the Russian Federation, as well as other regulations Public organizations promote the role of the expert community by engaging leading experts to develop expert proposals on health care policy improvement, obtain comprehensive and objective information on the implementation of state projects and programs, decrees and instructions, as well as help the state to overcome emergency situations. All of the above reflect the main role of public organizations in state policy making in the field of health protection and promotion.
