The high prevalence, burden and social impact of mental disorders, the lack of mental health care professionals and services around the world, the improvement and dissemination of digital technologies are the factors that have provided increased attention to the process of digital transformation in mental health care recently. The COVID-19 pandemic has increased the importance of remote interaction between different participants in the therapeutic process. Digital technologies in mental health care can bring significant advances in both clinical and scientific psychiatry. The article presents the most promising digital solutions for the diagnosis, therapeutic interventions and prevention of mental disorders. The technologies by which they are implemented are described, including computer programs, virtual and augmented reality, internet resources. The advantages and disadvantages of digital technologies usage in psychiatric practice are analyzed. The article described the main barriers that stand in the way of digital transformation in mental health, including insufficient scientific and practical support of the developed digital products, the lack of methodological foundations, the regulatory framework, and legislation.
DIGITAL MEDICINE
Digitalization – is one of the most promising trends of the healthcare system transformation, including mental health care. Telemedicine opportunities allow to improve health care system, to resolve the problem of the qualified specialist’s deficit and provide the remote territories with appropriate medical care, it also provides education possibilities for the mental health specialists from all over Russia offering continual training opportunities. Digital technologies can help substantially improve the science data collection in the field of mental health, remote access ensures patient enrollment from the different regions of the country, it allows to make the measures in the typical patient’s environment, data accumulated with the help of digital assistants open the new possibilities in the study of mental disorders. Digital behavior and behavior in the digital environment became the new research objectives in the field of mental health. Problem internet use with the behavior addiction development is in the research focus of the researchers aimed to elaborate early intervention and prevention programs. The use of the computerized programs, gamification, VR and mobile apps possibilities should become the essential part of the medical care provision in the field of mental health with the hybrid models implementation where the patient will have the option to choose individualized treatment program with the optimal ratio between digital and classic treatments options. Digital technologies should take their place in the prevention and mental health sustenance in the nearest future.
MEDICAL AND SOCIAL EXPERTISE
Respiratory diseases require the close attention of medical specialists at all levels of providing medical and diagnostic care to patients, as well as improving preventive measures. In recent years, there has been an increase in the incidence of respiratory diseases, an increase in the number of cases of temporary disability and the disability rate of the population. Disabled people due to respiratory diseases need complex treatment and rehabilitation to improve the quality of life. Purpose of the study. To study the organization of medical care for people with disabilities with respiratory diseases in 2019-2021. given the spread of COVID-19. Materials and methods. For the analysis, the primary registration forms of the State Budgetary Institution of Healthcare of the Republic of Bashkortostan “City Hospital No. 21” in Ufa and the Main Bureau of Medical and Social Expertise of the Republic of Bashkortostan were used. Results. It was found that all disabled people included in the study were hospitalized in 2019 (168.9 hospitalizations per 100 disabled people). The average duration of inpatient treatment was 17.6 days. All disabled people were consulted by general practitioners – 324.2 consultations per 100 disabled people, the least often consultation of a general practitioner was received by disabled people about bronchiectasis – 228.6 per 100 disabled people. Of the total number of disabled people, only 85.6 % were consulted by a pulmonologist (frequency 118.9 consultations per 100 disabled people). Despite the fact that all disabled people underwent certain clinical and laboratory examinations, their frequency was lower than that provided for by regulatory legal acts. During the COVID-19 pandemic, the level of hospitalization of people with disabilities has significantly decreased, the length of stay in the hospital has decreased, and the volume of consultative, diagnostic and rehabilitation assistance has decreased. Conclusion. The analysis carried out indicates the need to improve medical care for the disabled and to plan measures to increase the volume of medical and diagnostic measures during the outbreak of epidemics of infectious diseases.
FOREIGN EXPERIENCE
Improving the provision of medical care to patients with chronic non-communicable diseases should be based on objective indicators of the effectiveness of preventive measures and medical examinations. Such an indicator can be the incidence of acute myocardial infarction and cerebral infarction. Objective: to characterize the trends in the dynamics of the incidence of myocardial infarction and acute disorders of cerebral circulation in Turkmenistan. Materials and methods. A retrospective descriptive epidemiological study was performed. The dynamics, spatial and temporal characteristics of the incidence of myocardial infarction (ICD-10 code I21–I23), cerebral infarction (ICD-10 code I63) in the period 2016– 2020 in Turkmenistan as a whole and by administrative-territorial units were studied. Results. During the study period, Turkmenistan has seen a decrease in the overall incidence of acute myocardial infarction from 10.9 per 100 thousand in 2016 to 7.8 per 100 thousand in 2020. Currently, the highest level of total incidence of myocardial infarction is registered in Ashgabat and the Balkan Velayat (9.0 per 100 thousand people), the smallest is in the Mary Velayat (6.9 per 100 thousand people). There is also a decrease in the overall incidence of acute disorders of cerebral circulation. In the whole country from 2016 to 2020, the indicator decreased from 5.6 to 4.7 per 100 thousand population. At the level of individual administrative-territorial units, significant fluctuations in indicators were observed during the study period, and in the city of Ashgabat, an increase in the overall incidence of brain infarction. Conclusion. The decrease in the overall incidence of acute cerebral circulatory disorders and acute myocardial infarction indicates the high quality of the work of the national health system in the aspect of prevention of chronic NCDs. In some administrative-territorial units of Turkmenistan, the peculiarities of the dynamics of morbidity were revealed, requiring special organizational approaches.

Healthcare professionals regularly face high levels of professional stress, which can have a negative impact on their mental health. As demands on healthcare systems increase both in terms of the quality of medical care and the level of service provided, it is important for decision-makers to give due consideration to the psychological well-being of healthcare workers. This article will discuss the reasons for growing concerns about the mental health of medical professionals, including the consequences of the COVID-19 pandemic, the importance of addressing this issue, and steps that regulators can take to ensure better working conditions for physicians and nurses. By paying attention to the mental health of healthcare workers, together we can create more resilient and stable healthcare systems. New approaches may include the implementation of mental health strengthening programs, providing access to phycological training, as well as a review of the policy of medical organizations management, with a priority on the well-being of medical professionals.
