Abstract

The qualification requirements for the position of a medical psychologist are analyzed. The demand for the professional activity of a medical psychologist in the provision of psychiatric, narcological, neurological and general somatic care is shown. The knowledge, skills and abilities of medical psychologists are widely used in the providing medical care for diseases and behavioral disorders, in the consideration of forensic psychiatric examination. There is a tendency to further strengthening the interaction of medical psychologists with psychiatrists and psychotherapists. The features of undergraduate and postgraduate training in the specialty “Clinical Psychology” are highlighted. A comparative analysis of the staffing of medical organizations of the Russian Federation by medical psychologists was carried out, the unevenness of this indicator across the regions of the country was shown. The conclusion is made about the relevance of the adoption of the professional standard “Medical Psychologist” for optimizing the qualification characteristics of professional activity and creating an objective basis for building professional higher and postgraduate education programs in clinical psychology, taking into account the requirements of the employer. Prospects for increasing the role of medical psychologists in the organizations of the Russian Ministry of Health are associated with the inclusion of their functional duties in the Procedures for providing of medical care in general somatic practice and with the expansion of the number of universities providing training in the specialty “Clinical Psychology”.

Abstract

The article analyses current understanding of clinical psychology as a field of professional activity of psychologists in medicine and public health. It reveals the main traditional and new areas of activity of a medical psychologist in clinical and preventive medicine. Main directions and forms of clinical psychologist’ work in healthcare system is reviewed. The role of clinical psychologist as part of a multidisciplinary team is outlined and the main models of multidisciplinary teams in the field of medical rehabilitation are analyzed. We identified the main problems and the goals in this area, which solution is necessary to create an effective system for the provision of medical, psychological and social assistance to the population. The article shows that clinical psychology plays a key role within the multidisciplinary approach to assistance of patients and their relatives in specialized psychiatric or addiction treatment, psychotherapy, as well as in general medical practice. One of the main tasks of clinical psychology is considered humanization and personalization of the entire process of medical care, protection from inconvenient impact of technization of medicine on patient and doctor, narrow specialization of health services. Some of the organizational and legal barriers hindering full integration of medical psychologist into the healthcare system and effective realization of the goals of medical and psychological activities were highlighted. The prospects for the development of clinical psychology in healthcare in the theoretical, methodological, substantive and organizational-legal aspects are determined. It is emphasized that in order to realize the potential of clinical psychology, constant improvement of theory and methodology, the development of effective medical and psychological technologies for diagnosis, correction, and rehabilitation are necessary. And at the same time, it is important to create organizational and legal conditions for the full cooperation of clinical psychologist with specialists in other areas.

Abstract

The growing trend in the number of shootings cases in schools determines the need to take preventive measures to identify minors at risk of committing socially dangerous acts. The purpose is to determine approaches for interdepartmental interaction between specialized medical organizations and educational organizations in the aspect of minors auto- and heteroaggressive behavior preventing. Materials and methods. Behavioral indicators and psychological characteristics of adolescents who planned or committed attacks on educational institutions, minors with autoaggressive behavior who underwent complex forensic psychological and psychiatric examinations at the FSBI “NMIC PN named after V.P. Serbsky” of the Ministry of Health of Russia are analyzed. Results. In 2022, a new version of the «Methodological materials on the signs of deviations, the actions of education system specialists in situations of social risks and prevention of deviant behavior of students» was introduced into the educational process (methodological development “Prevention Navigator”). One of the new aspects is a block dedicated to identifying the risk of an attack on an educational organization (signs of the risk of committing a particularly dangerous act and a special algorithm of actions). The article describes aspects of the interaction of educational organizations with specialized medical institutions in the context of identifying such students and preventing socially dangerous behavior of minors. Conclusion. The current system of interaction between educational institutions and medical organizations specializing in the field of mental health of children and adolescents requires improvement and legal support

Abstract

Active development of scientific technologies and the digital transformation of the healthcare service promise a serious breakthrough in understanding the norm and pathology, assessing the risks of diseases or specific mental conditions, predicting their course and possible consequences. In other words, there are hopes that in the nearest future science allows modeling and predicting of complex human behavior at least in some contexts. The article analyzes the progress in the field of human behavior modeling in medicine and, particularly, in psychiatry, for which the explanation of behavioral disorders is the most relevant. 1175 publications were reviewed and 74 of them were selected for further analysis of the exiting methods for human behavior modeling in norm and mental pathology. The article highlights the possibilities of human behavior modeling and its most promising prospects. The possibilities of such modeling at the present time are limited. Most part of the created models have no sufficient verification and are unsuitable for solving real practical problems. In addition, science progress is far from explaining complex variants of human behavior, and it is unclear if it is possible to model such behavior using computer architecture, which is significantly different from biological. Various behavioral models are considered, that allow to solve specific practical tasks in psychiatry and healthcare system, and therefore seem to be the most promising.

Abstract

The relevance of the development of a preventive direction in the field of mental health care is beyond doubt. Mental health is an integral part and the most important component of the quality of health and includes not only the absence of mental disorders and various forms of disability caused by them, but also the state of mental well-being, in which the individual realizes his individual abilities, can withstand stress loads and be productive in everyday and professional activities. In this respect, the preservation of mental health is at the core not only of the well-being of the individual and his microsociety, but also of the functioning of society as a whole. Mental health care not only concerns the early diagnosis and effective treatment of mental disorders, but also focuses on broad aspects of population psychoprophylaxis and informational psychohygiene, contributing to the promotion of mental health and reducing the prevalence of the most relevant mental disorders. There is evidence of the positive impact of public health interventions on the prevalence of mental disorders. The general principles of the system of psychoprophylaxis imply the organization of three components: psychohygienic, psychoprophylactic and public mental health monitoring. The main directions of implementation of population psychoprophylaxis are: educational, socio-economic, environmental, humanistic and informational. Informational hygiene is a basic element of strengthening the mental health of the population, because a number of effects of informational influence can be predictors of mental ill-being of the population. Approaches to the assessment of informational content for its monitoring for destructive and manipulative content are proposed.

Abstract

The article contains examples of the impact of changes in the legislative provision of psychiatric care on the performance indicators of the psychiatric service and the mental health of the population. We analyze optimization of psychiatric care for patients with severe mental disorders, enabled by the legislation of hospital substitution, as well as existing barriers of providing care to patients with non-psychotic mental disorders. Such barriers include the stigmatization of psychiatry, on the one hand, and the lack of legislative support for the provision of medical care to patients with nonsevere mental disorders by non-psychiatrists, on the other hand. Such legislative peculiarities lead to a constant decrease in the detection of mental disorders, while the level of disability due to mental disorders remains stable in the population. In addition, the article provides a brief overview of the key changes in the Order of medical care for mental disorders and behavioral disorders implemented in 2022.
